Use of this form is limited to residents of the County of Palm Beach only.

The information on this form will assist the Sheriff's Office in identifying your bicycle in the event it is lost or stolen. If your bicycle is lost or stolen, report it to the Sheriff's Office immediately by calling the non-emergency number:

North/Central County: 688-3000 South County: 278-2644 West County: 996-1670

You can also register your bicycle by completing a paper Bicycle Registration Form at any Sheriff's Office Sub-station, or Satellite Office.

To Register:

Complete all of the boxes on the registration form.
Be certain to enter the serial number and other data in the boxes provided at the bottom of the form.You must be able to furnish the serial number if your bicycle is lost or stolen.
After completing the form, print out a copy for your own records.

Information will be electronically transferred to the Palm Beach County Sheriff's Office, Community Affairs Section.
